TOK? What does "Top of Keyway" mean?

Top of Keyway tesnion tools are designed to be inserted in the keywoay towards the direction of the pins. Since most lock pickers pick the cylinder open with the pins on top (because the cylinders are installed like this in America and some other countries), "Top of Keyway" has established itself as the term for this tensioning method.
